Thirty years after the Kuchisake Onna urban legend first started, the slit-mouthed woman returns to abduct children and bring them back to her hideout. Schoolteacher Matsuzaki knows her true identity and teams up with fellow schoolteacher Yamashita to put an end to her reign of terror. Unfortunately she is able to possess the bodies of mothers around the town, making killing her nearly impossible.
